HISTORY
Ten year old Wayne Franklin and his six year old sister Amina grew up with a father on the run from the law. When their father decided he would end their lives to protect them, rather than going to prison, Wayne turned his father's gun back to him, killing him. Fearing he would go to jail now, Wayne made his sister promise to tell the authorities their father killed himself.
Despite their early lives, the Franklin siblings grew up to become successful doctors. Wayne would later discover he had Parkinson's disease and his hands were becoming too unsteady for surgery. To pursue his work as a doctor, Wayne threw himself into the development of the I-Gore device... - Batman #662

Wayne Franklin as Grotesk used the I-Gor device in his quest for vengeance. It appears to have been weaponized as it is unlikely a surgical instrument would have a flame thrower. The I-Gor could also project hypodermic needles of various paralytic drugs, with the ability to deliver fatal doses as well.
